We’ll be happy to help, but as we receive a high volume of enquires daily, do allow us up to 24 hours within our operating hours to respond.

Making a reservation
How can I make a booking?
You can make an online booking via our reservations portal.
If there are no available slots on your desired date, you can choose the “Join the Waitlist” option for the same date. This does not guarantee an upcoming reservation, but places applicants next in line for any table that become available for their group. We will will reach out via the contact details provided in the waitlist applications once—and only if—we have a suitable table for your group.
I can't seem to get any available slots in your booking platform or through the waitlist. What are my other options?
We occasionally share any last minute availability for same- or next-day reservations (after exhausting the wait list) via our social media stories. Follow us on Facebook or Instagram and subscribe to our updates to get notified on the fly. You may also wish to contact us in the afternoon via Whatsapp to check on any last minute updates to the evening’s availability.
If all else fails, we open for weekend lunch on Friday and Saturday (12pm–2.30pm) where we operate a no-reservations policy and accept walk-in guests only. A slightly scaled down version of our a la carte menu will be served, view it here.
How often do you release your booking slots?
We release our bookings by the quarter of each year, which we will release for the next quarter in the middle mark of each quarter. You can follow us on Facebook and Instagram and get subscribe to get notified when we announce the booking launch date days before on our social media stories.
As we have been experiencing high volume of bookings with each release, this is done to allow more people a chance to be able to book a table with us.
Why is there a credit card authorisation required to confirm a booking?
A credit card authorisation of 50 SGD is required to confirm a booking, which can be made via our online reservations portal. The fee will only be charged in the event of no-shows or cancellations less than 24 hours away from the reservation time. This is done to best and fairly facilitate our high volume of reservations with our limited seating capacity.
I live nearby and am flexible on dates. Is there a way to get a table?
We’re always happy to see our neighbours pop by! If you live, work or play nearby, are flexible on dates and a shorter notice period, drop us a note on Whatsapp to let us know your contact details and preferences and we’ll reach out if we have a suitable last minute table for you.
Why can't I make a reservation for weekend lunch?
Our weekend lunch is walk-in only, and cannot be booked in advanced. This allows for a more relaxed lunch affair and an opportunity for us to turn more first-time guests into friends.
Things to note
Do you have a dress code?
Yes, we operate a dress code of smart casual. Tailored shorts and sandals are okay. Gym shorts and flip flops are not. You are welcome to come as you are for weekend lunch.
What is your corkage policy?
We take great pride in our selection of Italian wines and aperitif with over 150 labels spanning the country’s 20 wine regions—with gems from around the old and new world—that are fitting companions to our menu of regional specialties and handmade past.
Our corkage policy is 1-for-1, buy one of ours and we’ll open one of yours. Our in-house sommelier will be more than happy to recommend a bottle to impress and delight.
I am coming with a family with kids. Do you have a minimum age for diners?
We welcome all families with kids 12 and above for our dinner service. Families with kids of all ages are welcome to join us for weekend lunch every Friday and Saturday (12pm–2.30pm).
Can I bring a birthday cake?
We encourage guests to celebrate their birthdays with an array of our desserts, inspired by each season across different regions of Italy. that are freshly made in-house every day. However, if you wish to bring your own birthday cake, a plating fee of 35 SGD will be implemented to the bill.
Are you pet-friendly?
Woof. Our charming outdoor patio overlooking the quaint Tembeling road, sheltered by wide, sprawling leaves, are the perfect date spot to bring your furry friends. Let us know in advanced in your reservation notes so we can seat you accordingly and have a water bowl ready.
Do you offer take-out or delivery?
We currently do not offer take-out or delivery. Our dishes, especially the handmade pasta, are meant to be enjoyed in Forma itself.
Things to note
I'd like to amend my booking time/group size. How do I go about doing so?
You can let us know your booking code and requested amendments via Whatsapp and we will try our best to accommodate. However, this is subject to our seating capacity.
What happens if I have to cancel less than 24 hours away from my reservation?
Once you are 24 hours away from your reservation time, you will no longer be able to cancel via our online reservations portal and have to reach out to us to do so manually. If the reservation is cancelled less than 24 hours away or didn’t turn up without a valid reason, the authorised sum of 50 SGD will be charged to the credit card on record that was used to confirm the reservation. Multiple cancellations from the same guest, however, will be recorded and eventually blacklisted.
What would your recommend to order for the best experience of Forma?
We know it’s tempting to come just for the pasta and eat the entire season’s worth in one seating, but that’s no way to spend a meal: on a mission.
Instead, fill your table with dishes and wines that can take you on a journey through Italy’s astounding culinary regional diversity, surround it with familial ties and warm friendships, and celebrate the Italian way of “La Bella Vita” kindled around convivial dining occasions.
For dinner, we recommend a family-style meal of 2-3 antipasti, 2 primi, 1 secondo, 2 dolci and a bottle of vino to be shared between every two guests.
For weekend lunch, we recommend at least one antipasto, one primo, and one dolce per guest for a satiating lunch. A bottle of vino and secondo to share if you’ve got time to spare.
